Whats the reasons for pain in chest and having hard time to breathe?

Hi, I'm a 16 year old girl and I have pain in chest, hard to breathe and sometimes hard to swallow. I also get a weakness in my legs that I can't walk and I try and hurry to sit down because it feels as I would fall down, can you please help with what's wrong?
